At Ayong Health Care LLC, we endeavor to provide individualized services for all of our clients. Acknowledging that each person is unique and has different needs, we design our service plans in accordance with each person’s care and support requirements.

Our services include:

Residential Support Services
This is a group home option for individuals that have behavioral or medical condition issues which require constant supervision for their safety. It is staffed 24/7.

Supported Home Living
Individuals reside in their home or family home. Skilled staff provides care and support with activities of daily living (ADLs).

Supervised Living
Qualified individuals living together who do not require constant supervision – either medical or behavioral). Skilled staff members are available 24/7 to respond as needed.

Companion / Foster Care
These are individuals living in their family homes and are cared for by a family member, a provider, or a CNA.

Case Management
All of our clients will have assigned case managers who can monitor their plan of care. Case managers are also responsible for creating the personal service plan, and they have to document all services rendered to our clients as required by the HHS.

Licensed / Certification Services
Nursing, Medical Services, Dental, Vision, Hearing Services, Psychiatry, Psychology, Day Programming, and Public School.

Non-Medical Services
Our staff is available 24 hours a day, 7 days a week. Aside from the services mentioned above, they can also provide meal preparation assistance, housekeeping services, transportation to appointments, laundry, grooming, and assistance with daily living skills. They can also prepare fun activities like outdoor games, playing cards, craft work, puzzles, and field trips.

Day Habilitation
In Day Hab, we offer health and social services. We encourage individuals to function independently as possible. Our direct staff care develops services and plans which are responsible to each individuals needs and abilities. There are always fun activities, arts, crafts, playing board games, bingo, watching movies and much more. Community based programs, school, work based on every individual’s ability. Our skilled registered nurse is always on site for providing oversight to ensure medication safety, advocating proactive health and ensure safety for all individuals which is paramount.

Respite Care
Is a short term care break that is provided to care givers who take care of their love ones in their homes and need a break from their caring role . we provide services for arranged, and unplanned breaks, regular weekly breaks, short holidays or emergencies. our services are available ,hourly, daily, weekly, or monthly in the care giver’s home, our day care centers or residential care facilities.
- PT Services
Our physical therapy services aim to promote better musculoskeletal skills and enhanced mobility. Therapy sessions can be conducted in your home or any living environment of your choice. Some treatments include exercise programs, body mechanics rehabilitation, a range of motion exercises, and fall risk reduction. - OT Services
An Occupational Therapist can develop interventions for you to recover your functional skills needed for your day-to-day activities. Therapy sessions are aimed at providing you with coping skills when your mobility or physical functioning has been affected by a recent surgery, a newly diagnosed illness, or a result of an injury from accidents.

Medical Services
Our skilled registered nurse are available to trained our care staff team and provide direct services including:
- Medication administration and reminders
- Education of medication therapy and safety
- Monitor blood pressure
- Monitor blood sugar, diabetic care
- Wound care, preventing, treating pressure injuries
- Infusion therapy, antibiotics, hydration.
- External nutrition, peg tube, ng tube, j-button feeding,
- Parental nutrition, ppn, tpn, lipids
- Incontinent care, foley care
- Airway management, trach care, ventilator care, oxygen theray, bipap, cpap, apnea monitor.
- Ostomy care, ileostomy, colostomy, nephrostomy,
- Catheter care

Note: All services are provided under the direct supervision of skilled and experienced professionals following each person’s individualized plan of care.
Eligibility Requirements
Here are our eligibility requirements at Ayong Health Care LLC:
- Must have the diagnosis of intellectual disability or related condition
- Must have a current level of care or level of needs
- Must have a current individual plan of care
- Must be currently receiving SSI with Medicaid
- Must have HCS slot offered to individual if not currently enrolled in HCS services
- Must be able to do private pay if they do not have HCS funding
